Why not self-host?
You already know the software works. The problem is keeping it running at 2 AM when a radiologist needs priors.
Complexity
Orthanc needs PostgreSQL, the DICOMweb plugin, Lua scripting for routing, and storage that scales. dcm4chee needs WildFly, OpenLDAP, and a correctly tuned PostgreSQL. We configure and maintain all of it.
Compliance
Patient imaging data is special-category under GDPR Art. 9. You need a DPA, documented TOMs, audit-ready access logs, and encrypted backups with tested restores. We provide all of it — not a checkbox, actual documentation you can hand to your DPO.
Reliability
When your PACS is down, clinicians can't pull priors and reports don't get written. We run automated backups every 6 hours, 24/7 monitoring with alerting, and tested failover. 99.9% uptime SLA.
